Urban Health Plan Expands eClinicalWorks Relationship for Care Initiative
HIMSS Davies Award-winning Center Selects eClinicalWorks Care Coordination Medical Records Solution to Assist with Accountable Care Organization Goals
WESTBOROUGH, Mass.—December 20, 2012—eClinicalWorks®, a market leader in ambulatory clinical systems, today announces that New York’s Urban Health Plan (UHP) is expanding its relationship with the company to include eClinicalWorks Care Coordination Medical Record™ (CCMR) for advancing its Accountable Care Organization (ACO)-related objectives, coordinating care across the continuum as well as evaluating population health and quality improvement activities. UHP’s more than 100 medical providers currently utilize eClinicalWorks comprehensive electronic health records (EHR) solution and the organization received a 2009 HIMSS Davies Award for its EHR implementation.

A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payor claim feeds, including Centers for Medicare & Medicaid Services (CMS), to provide cost and utilization for services delivered to patients.
Included in CCMR, eClinicalWorks Community Analytics™ uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage Population Health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, for including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.

UHP was re-recognized as a Level 3 Physician Practice Connections – Patient-Centered Medical Home (PPC-PCMH) from the National Committee for Quality Assurance (NCQA) for the 2011 standards. Level 3 recognition is the highest awarded by the NCQA.
