Medicare Accountable Care Organization Chooses eClinicalWorks Care Coordination Medical Record Solution to Advance Accountable Care Organization Goals
WESTBOROUGH, Mass.—December 16, 2013—eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announced that the Louisiana Senior Care Coalition has selected the eClinicalWorks population health management solution, eClinicalWorks Care Coordination Medical Record™ (CCMR), to advance its physician-led Accountable Care Organization (ACO)objectives. With 25,000 members in the Baton Rouge area, this agreement will achieve goals that include coordinating care across the continuum, evaluating population health, and quality improvement activities.

With 45% of its target population experiencing major health concerns, LSCC formed the ACO to coordinate care between patients and members of the healthcare team in order to strengthen the physician-patient relationship for better care and reduced costs. LSCC participates in the Medicare Shared Savings Program, which was created by the Centers for Medicare & Medicaid Services (CMS) to facilitate coordination between providers for improved quality of care to Medicare fee-for-service beneficiaries and reduce unnecessary costs. eClinicalWorks CCMR will aid LSCC in achieving the quality measures and performance metrics set under ACO and Patient-Centered Medical Home guidelines.
