Greater Baltimore Medical Center Partners with eClinicalWorks for ACO Initiative
One of the Largest Community Hospitals in the Mid-Atlantic Selects eClinicalWorks Care Coordination Portal and eClinicalWorks Community Analytics
WESTBOROUGH, Mass.—February 27, 2012—eClinicalWorks®, a market leader in ambulatory clinical systems, today announces that Greater Baltimore Medical Center (GBMC), one of the largest community hospitals in the mid-Atlantic, has expanded its relationship with the company to include eClinicalWorks Care Coordination Portal and eClinicalWorks Community Analytics. This will aid GBMC in meeting its objectives as an Accountable Care Organization (ACO), coordinating care across the continuum as well as evaluating population health and quality improvement activities.
This expansion will additionally assist GBMC in working with its practices in becoming National Committee for Quality Assurance (NCQA)-recognized Patient-Centered Medical Homes (PCMH). Both ACO and PCMH delivery models coordinate care between patients and members of the healthcare team with the goal being to help strengthen the physician-patient relationship, leading to better care and reduced avoidable costs over time.

eClinicalWorks Care Coordination Portal is designed to provide visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. eClinicalWorks Community Analytics delivers reporting, alerting and messaging capabilities necessary to manage population health. Reports can be run at the community level to determine and monitor adherence of clinical process and outcomes, for including medication recalls and chronic disease management. Both solutions work together for a collaborative care framework.
