Organization Successfully Utilizing eClinicalWorks CCMR in Three Ohio and Indiana Accountable Care Organizations that Represent 51,663 Members
WESTBOROUGH, Mass.—April 23, 2014—eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announced American Health Network (AHN) is live using eClinicalWorks Care Coordination Medical Record® (CCMR) for population health management to manage three Accountable Care Organizations (ACOs) – American Health Network of Ohio Care Organization, Franciscan AHN ACO and Indiana Care Organization. These ACOs have a total of 51,663 Medicare members and 320 providers across Ohio and Indiana. As part of this relationship, eClinicalWorks provided the technology and resources to assist all three ACOs with their successful submissions for Group Practice Reporting Option (GPRO) in 2013, a requirement for Medicare ACOs.

The ACO care delivery model coordinates care between patients and members of the healthcare team with the goal of helping strengthen the physician-patient relationship, leading to better care and reduced avoidable costs over time. The AHN ACOs participate in the Medicare Shared Savings Program, which has been created by CMS for facilitating coordination between providers to improve the quality of care for Medicare Fee-For-Service (FFS) beneficiaries, improve patient satisfaction and reduce unnecessary costs. eClinicalWorks CCMR is a Web-based, population health management platform for supporting ACOs along with other shared-savings models and will aid AHN, and its partner ACOs, in meeting its quality measures and performance metrics.

Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management.
