Reinventing the Post-Visit Process

Starting in 1878, Thomas Edison and his associates began searching for a way to replace the gas light with an inexpensive electric one. As experimentation began, Edison had to go through experiment after experiment until finding the most effective solution.
“Before I got through,” Edison said, “I tested no fewer than 6,000 vegetable growths, and ransacked the world for the most suitable filament material.”
Like Edison, who understood the necessity for reinvention to accomplish his goals, providers in the healthcare industry have recognized the need for reinvention during the post-visit process.
Care doesn’t need to end when the patient visit is over. Utilizing the right tools for the job could help providers better identify and meet patient’s needs to promote better outcomes.
Improving your post-visit process could start by:
- Empowering patients by encouraging them to download lifestyle and health management apps
- Implementing Population Health tools for Chronic Care Management/Transition Care Management
- Sending satisfaction surveys and reminders to patients
- Encouraging the use of wearable health devices to gather patient data
