The following reports reflect payments and transfers of value, in cash or in kind, directly and indirectly, to any physician, physician practice and any other individual or entity involved in providing, directly or indirectly, health care items or services to Federal health care program beneficiaries, except for a physician or other individual who is a bona fide employee of eClinicalWorks.
