Northern Ohio Medical Specialists Live with eClinicalWorks for Population Health
Physician-led, Medicare Accountable Care Organization Utilizing eClinicalWorks Care Coordination Medical Records Platform
WESTBOROUGH, Mass.—January 5, 2015—eClinicalWorks®, a leader in ambulatory healthcare IT solutions, today announces that Northern Ohio Medical Specialists (NOMS) Accountable Care Organization (ACO) is now live with eClinicalWorks Care Coordination Medical Records (CCMR) for population health management. NOMS ACO participates in the Medicare Shared Savings Program, which the Centers for Medicare & Medicaid Services (CMS) established to facilitate coordination between medical providers to improve the quality of care for Medicare beneficiaries and reduce unnecessary costs. The ACO care delivery model can also help strengthen the physician-patient relationship.
“The mission of NOMS ACO is to improve and maintain the health of all people in our community,” said Louis Ralofsky, M.D., CEO and president of Northern Ohio Medical Specialists ACO. “This is achieved by delivering the highest-quality, personalized patient care in a caring and compassionate environment. eClinicalWorks CCMR will aid us in our care goals by delivering a comprehensive population health platform that includes dashboards to identify gaps in the care. Its closed loop analytics along with care planning technology will make results actionable, resulting in quality improvement.”
eClinicalWorks CCMR will provide NOMS ACO with visibility into a patient’s care across all settings, facilitate smooth transitions of care, engage patients and providers in preventative care, improve outcomes among populations and reduce costs. The platform provides a rich, intuitive approach to clinical, fiscal and predictive analytics along with risk stratification and quality measure reporting. A vendor-neutral, open network securely connects ambulatory EHRs with hospital in-patient systems and payer claim feeds, including CMS, to provide cost and utilization for services delivered to patients.
“NOMS ACO focuses on developing and supporting healthcare practices recognized for exceeding patient expectations,” said Girish Navani, CEO and co-founder of eClinicalWorks. “Utilizing eClinicalWorks CCMR gives NOMS ACO access to powerful analytics and care coordination on a single platform. Its providers will gain deeper insight and understanding of patient populations and discover new opportunities in patient care.”
Included in CCMR, eClinicalWorks Population Health Management Analytics uses closed-loop analytics to deliver reporting, alerting and messaging capabilities necessary to manage population health. Dashboards can be shown at the community level to determine and monitor adherence of clinical process and outcomes, including medication recalls and chronic disease management. Care coordination and analytics work together for a collaborative care framework.
